What this calculates
This is the reconstitution calculation run backwards. Instead of choosing a diluent volume and seeing what concentration results, you state the concentration you want and it tells you how much liquid produces it. That is the more useful direction when you have a reason to prefer a particular strength — usually because it makes the syringe reading land on convenient marks, or because it keeps a full dose within the volume the barrel can hold.
How it works
The diluent volume is the amount in the vial divided by the target concentration. A 10 mg vial at a target of 5 mg/mL needs 2 mL. As with the forward calculation, the dissolved solid adds a little volume of its own, so the true concentration comes out marginally below target — a difference far smaller than a syringe can measure at these quantities, and one this calculation ignores. The per-unit figure and the total unit count follow from the syringe scale, and are shown because the point of choosing a concentration is usually to control what those numbers look like.
Worked example
A vial holds 10 mg and you want a 5 mg/mL solution: 10 ÷ 5 = 2 mL of diluent. On a U-100 syringe that fills 200 marks, and each mark holds 5,000 ÷ 100 = 50 mcg. Suppose you would rather each mark be worth 25 mcg. That means a 2.5 mg/mL solution, which needs 10 ÷ 2.5 = 4 mL — double the diluent, half the strength, twice as many marks for the same quantity drawn.
Common mistakes
Choosing a target concentration without checking the resulting volume against the vial. A vial has a fixed capacity, and asking for a dilute solution can require more liquid than will physically fit — the arithmetic will happily return a number larger than the container. Second, forgetting that a lower concentration means drawing a larger volume for the same dose, which can exceed what a small-barrel syringe holds. Third, mixing units between the vial field and the target field; the target is in mg/mL, so a vial entered in micrograms is converted first.
Common questions
- Is a more dilute solution more accurate?
- Up to a point. Spreading a dose across more syringe marks reduces the effect of a small reading error, but only until the volume needed exceeds what the barrel holds. There is a practical middle rather than a best answer.
- What if the volume is more than the vial can hold?
- Then that concentration is not achievable in that vial. Choose a higher target concentration, and check the calculated volume against the vial capacity before adding anything.
